Case Management

Francis Homes participants will be required to maintain a third party, community based case manager for the duration of the lease period. Participants will be expected to participate in the program requirements maintained by the third party case management partner. Failure to maintain case management support will result in nonrenewal of future leases and an unsuccessful exit from the program.

Shelter Development, Inc. will develop and maintain partner relationships with known Case management providers. For Shelter Development consideration, case management will refer to the services offered by the partner agency which include. 

-A case management methodology that meets the standards of Case management outlined by the Global Social Service Workforce Alliance 

-A developed, written, case management plan.

-Supportive Case management interventions that promote financial independence, resource and referral attainment, education and/or employment support and social skill building.

-Ongoing and regular contact either in person, or via phone.

-Case management partner relationships will be formalized with Shelter Development, Inc. via written Memorandum of Understanding (MOU).
